Insurance Options

Bloom Acupuncture & Integrative Medicine is not in-network with any insurance companies and does not accept HMO plans. However, some patients with PPO insurance plans that include out-of-network acupuncture benefits may be eligible for reimbursement from their insurance provider.

If you have a PPO plan, we can help verify your out-of-network acupuncture benefits to determine whether your plan may provide coverage for treatment related to your condition.

PPO Out-of-Network Benefits

If your insurance plan includes acupuncture coverage, you will typically pay for your treatment at the time of service and then submit documentation to your insurance company for possible reimbursement.

We can provide the appropriate documentation to assist with this process, including:

• An insurance Superbill
• CMS-1500 reimbursement documentation
• Treatment and billing information required by your insurance carrier

The amount reimbursed, if any, will depend on your individual insurance plan, deductible, coverage limits, and applicable out-of-network benefits.

HSA Payments

If you have a Health Savings Account (HSA), you may be able to use those funds toward eligible acupuncture and healthcare services. Coverage and eligibility can vary, so we recommend confirming your specific benefits with your HSA administrator when necessary.

Lab Testing & Insurance

Some treatment plans may include or recommend laboratory testing. Lab testing is not covered by many insurance benefit packages, so if laboratory testing is being considered, we strongly recommend checking your individual plan for lab coverage before proceeding.

Any applicable laboratory fees may be separate from your acupuncture or integrative medicine treatment costs.

Please Verify Your Benefits

While our office can assist with verifying PPO out-of-network benefits, insurance plans and benefit details can vary significantly.

We strongly recommend that patients also contact their insurance company directly to verify their benefits before beginning treatment.

When contacting your insurance provider, consider asking:

• Does my plan include out-of-network acupuncture benefits?
• Is acupuncture covered for my specific condition?
• Do I have an out-of-network deductible?
• Has my deductible been met?
• Is there a limit on the number of acupuncture visits allowed each year?
• What percentage of the treatment cost is reimbursed?
• Is a referral or preauthorization required?
• Is laboratory testing covered under my plan?

Insurance verification is not a guarantee of payment or reimbursement. Final coverage decisions are made by your insurance provider according to the terms of your individual policy.
