* Note: We do not accept HMO plans, as we are not In-Network with any insurance companies. We can verify your benefits for PPO out of network plans, and if your benefits package covers Acupuncture for your condition then we can provide you with a Super-Bill and CMS1500 insurance reimbursment form.
* Lab testing is not covered within many benefit packages, please check on lab testing coverage when verifying benefits, (if lab testing is desired).
* It is ALWAYS a good idea for the patient to get verification of benefits from their insurance company as well as the provider.
